Deena Sandos, MSW, is a Senior Manager of Special Projects at the American Geriatrics Society (AGS). Ms. Sandos manages the AGS CoCare® portfolio which is a suite of programs helping to embed geriatrics expertise in broader care for older adults. This currently includes AGS CoCare®: Ortho, a Geriatrics-Orthopedics Co-Management Program, and AGS CoCare®: HELP, a Hospital Elder Life Program.

Charles Eleosida, MBA, CAVS

Charles leverages his academic knowledge and extensive volunteer experience to mobilize hospital volunteers across both inpatient and outpatient programs, creating meaningful impacts on the lives of patients, families, and Southside communities. Charles Dominic Eleosida is a Program Manager at UChicago Medicine where he strives to elevate the human experience through volunteerism. Working under the Urban Health Initiative, he is celebrating his 6th year leading volunteers. With a degree in business management focused on organizational leadership and economics, Charles brings a wealth of experience from his previous work in talent acquisition and social entrepreneurship projects that empower communities through access to microfinance resources. Service has always been central to Charles’s life, from volunteering at a soup kitchen in high school to leading service projects with Alpha Phi Omega in college. He has worked with organizations such as Shriners Hospitals for Children, Human Connections, and Investors in Mexico. He continues his involvement with nonprofits like Cradle to Crayons and the Greater Chicago Food Depository. Lauren Gleason, MD, MPH, CMD, AGSF
Associate Professor of Medicine
Medical Director Post-Acute Care Continuum, UChicago Medicine
Dr. Lauren J. Gleason is an Assistant Professor of Medicine in the Section of Geriatrics and Palliative Medicine. She completed her medical degree at the University at Buffalo School of Medicine, followed by her residency in internal medicine at the University of Rochester and a geriatrics fellowship through the Harvard Multicampus Program. She also completed an MPH in clinical effectiveness at Harvard T.H. Chan School of Public Health. She is a geriatrician at UChicago Medicine and also cares for patients at UChicago Medicine Senior Care – South Shore and Symphony of South Shore where she is co–medical director. She also serves as the medical director on the inpatient geriatrics consult service. She is the project director of a HRSA–funded Geriatrics Academic Career Award (GACA). Through this project, she leads geriatrics educational initiatives for health care professionals, students and community members around the 4Ms (Medications, Mobility, Mentation, Matters Most). Her interests involve development and implementation of IHI Age–Friendly QI initiatives and improving quality of care for older adults in the health system. Currently, she is working on implementing universal delirium screening in the hospital system. In her role at Symphony of South Shore, Dr. Gleason led the nursing home team and staff through its COVID–19 management. She collaborated with UCM infectious disease and geriatrics faculty, geriatrics fellows and others, in implementing novel cohorting and medical management of COVID–19 and developed communication templates to assist families during the on–going visitor restrictions. Her work resulted in 5 publications. She has also been involved in clinical teaching through the AHRQ–IHI COVID–19 Nursing Home.

Lyn Weinberg, MD
Director, Division of Geriatrics
Allegheny Health Network
Lyn Weinberg, MD, is a geriatrician and the Director of the Division of Geriatrics, specializing in comprehensive care for older adults. She has a special clinical interest in Alzheimer’s disease and related dementias, delirium management and prevention, frailty, polypharmacy, and providing support for caregivers. Dr. Weinberg also focuses on healthy aging and preventive care, offering comprehensive geriatric assessments and memory disorder evaluations. Dr. Weinberg earned her medical degree from Jefferson Medical College, Thomas Jefferson University in Philadelphia, Pennsylvania. She completed her residency in internal medicine at Temple University Hospital and a fellowship in geriatric medicine at the University of Pittsburgh Medical Center. Dr. Weinberg is certified by the American Board of Internal Medicine in Geriatric Medicine and Internal Medicine. She values a holistic approach to geriatric care, incorporating the “4Ms” framework of age-friendly health care, which emphasizes mentation, mobility, medications, and “what matters” most to the patient.
